Main purpose of the position
To oversee and manage injured employees and require post-hospitalization or discharge care, to ensure they receive appropriate care, support, and assistance throughout their recovery process.
Job Responsibilities
- Coordinates implementation of case management programme interventions.
- Provides technical support on program designs, protocol formulations and health and policies.
- Monitors operational activities within designated area to ensure timely reporting.
- Coordinates care and support of injured, hospitalised, and discharged employees
- Facilitates the management of cases of injured employees, including coordinating medical treatment, rehabilitation, and return-to-work plans.
- Coordinates involvement of physical fitness facilities for improved employee wellness and health.
- Liaises with external service providers, including health professionals and medical facilities to optimize delivery.
- Provides technical support on case management program designs, protocol formulations and wellness policies.
- Monitors operational activities within designated service area to ensure timely reporting.
- Providing immediate support and intervention in crisis situations, such as medical emergencies.
- Quick assessment of the nature and severity of the injured, including the individual’s safety and immediate needs.
- Manage psychological, social, and mental health issues of affected employees within the Corporation to empower them.
- Coordinates provision of emotional and social support to employees and employees’ families during difficult times (death, IODs).
- Conduct hospital and home visits to the injured employees and their families.
- Monitors implementation and compliance of the Corporation’s Employee, Health and Wellness budgets and controls expenditure against authorized budgets.
- Contribute to the budgetary process of healthcare financing with focus on insurance coverage,
- Facilitate reimbursements for the Corporation and implement financial assistance programs for employees
- Manage injured employees’ illness absenteeism and facilitates referral to the relevant internal stakeholders
- Liaise with medical facilities & line management to enable processing of medical case documentation.
- Analyse ill health trends and advise relevant internal stakeholders (SHER, line management and HR-Services) on corrective measures.
- Discharge Planning: Knowledge of discharge planning processes and resources to ensure smooth transitions for patients from the hospital to other care settings.
- Coordinates provision of appropriate medical assistance (treatment and rehabilitation) followingemployees injury incidents
- Coordinates referral of injured employees to varying medical facilities for further management.
- Coordinates the promotion of health and safety through information, education, campaigns, and communication sessions educate employees on electrical safety measures to prevent future incidents and promoting a culture of safety within the organization.
- Designing health and wellness messages with focus on case management, medical insurance,and disability claims for employees (standard operation procedures, fliers, pamphlets, posters).
- Represent the Corporation at health stakeholder holder meetings (Risk Underwriter meetings, National and International Conferences)
- Implement and maintain up-to-date employee’s case management records.
- Medical Documentation: implement and observe medical record documentation practices and standards to ensure accurate and complete documentation of patient care.
- Provide advice on the implementation of organisational and national health policies and
- coordinates education of staff on these policies.
- Contributes to the development of healthcare response plans
Qualifications
- Minimum of a Degree in health administration or Nursing or Psychology or Physiotherapy or any health-related field.
- Postgraduate Certificate in a related field would be an added advantage.
- Membership to a professional Society/body would be an added advantage.
Experience
- A minimum of five (5) years post qualification experience in case management particularly in a healthcare setup and experience in workers compensation, disability management or similar.
- Two of the above should have been in an industrial environment.
